Advanced Hip Injury Treatments


Once your medical professional has diagnosed your condition, you can begin the most effective treatment that is appropriate for your hip injury. There are a variety of options available depending on the severity of your injury.

Inflammation - one of the first symptoms of soft tissue damage in the hip, is typically the result of one or more of the following conditions:

  • overuse of an area of the hip - often due to overcompensation or SLS (short leg syndrome)
  • a traumatic injury to the hip
  • tendon damage in the hip (tendinitis, tenosynovitis, tendonosis)
  • bursitis
  • hip muscle spasm
  • a strained hip muscle
  • soft tissue tear(s) in the hip
  • or an impingement in the hip

In most cases, the above conditions would be considered by physicians and specialists as non-surgically necessary. It is generally understood by doctors and surgeons, that surgery will introduce more scar tissue into the any already damaged tissue. This added scar tissue will be problematic, requiring more PT and conservative treatment options post-surgery. If not dealt with properly, your hip could end up in worse condition than before the surgery! This is why surgery is only performed as a last resort.

For cases that do not require immediate surgery, a protocol of conservative treatments would be recommended for a significant period of time (typically 3 months minimum). Surgery is typically only considered for such cases after conservative treatments options have been undertaken for a period of time and little or no improvement has been shown.

Non-Surgical Hip Treatments

In most cases, the goals for conservative treatment of a soft tissue injury would be:

  • decrease/eliminate swelling
  • relieve pain and stress on the hip
  • correct any bio-mechanical dysfunction - if one exists
  • and finally.. restoring strength and mobility to the hip

Allowing your hip to rest is recommended following injury, however, some careful movement is required to prevent the joint from freezing and losing range of motion. See your doctor or physical therapist for stretches that will not cause further injury to your hip. Avoid activities that may have caused the injury or irritation and begin cold treatments as soon as possible via use of a Cold Compress or Ice Pack.

The trick to healing your injury and getting your hip back in the best possible condition you can, is getting it to heal with as little scar tissue as possible. Scar tissue can bind tendons, muscles and ligaments together, limiting the movement of these tendons and ligaments that were once elastic and allowed the hip its large range of motion. As a result, movement of the entire joint is limited making it difficult to walk without a limp or do basic tasks such as climb stairs. If left untreated, the hip may 'freeze' further, making movement even more problematic. Even with a best case scenario recovery, there is typically less elasticity in previously injured tendons, muscles and ligaments. However, if you listen carefully and adhere to your physicians recommendations, you should experience minimal scar tissue build up. With minimal scar tissue, your chance of re-injury or chronic hip conditions later on is much lower than average.

Shortened muscles and tendons due to lack of use is a real problem during recovery. When you start to feel better and use the joint, the shortened muscles and tendons get over-strained, introducing small microtears. These microtears start the formation of scar tissue, which unlike muscle or tendon, is completely inflexible. Once there is scar tissue in there, the tears will continue; eventually you've got a serious problem with a joint that does not flex as much as it did before and this starts to become a chronic condition. You'll know you have scar tissue when you get shooting pain if you stretch your joint too far... it is something that physical therapist's often deal with (ie. shoulder manipulation).

To prevent this, we recommend using the TShellz Wrap before undertaking any significant activity, as warming treatments are intended to increase the flexibility of muscles, tendons and ligaments. More frequent short breaks (and even stretches) at work are usually very helpful, as this allows your body a chance to relax and take some strain off your stressed soft tissue.

It is important to speak with a medical professional or occupational therapist to get more information about proper equipment, procedures and tools necessary for the specific activity you participate in or the office/work setting you work in during your normal daily life.


Lifelong Health

The best way to manage the health of your body is to understand the tools for a healthy lifestyle. You may have to modify some of your activities or movements during your recovery to ensure the long term health of your lower back and hip. Most often, more serious soft tissue injuries are developed after ignoring your body's warning signs. Masking pain and immobility or continuing strenuous repetitive movements cause micro tears to form in your tendons. If your body is not given a chance to heal, scar tissue will form. This weak, inflexible tissue will cause chronic tendonosis symptoms that include constant pain and functional restriction.

Most cases of minor soft tissue injuries are completely treatable by conservative therapies. If you want to continue to enjoy your activities, sports, or have a physically demanding job, committing yourself to a conservative treatment routine, as soon as possible, will increase your chances of a full recovery.

Conservative Treatment Stages for Soft Tissue Injuries of the Hip, Leg and Lower Back

Your initial treatment should involve decreasing any swelling, relieving any pain and stress on the hip, correcting any bio-mechanical dysfunction, and then restoring strength and mobility to your injured connective tissue. It's also very important to follow any instructions you receive from your physician.

Avoid activities that may have caused the injury or irritation and begin cold compression treatments as soon as possible.



Conservative Treatment Step 1:
Reduce The Initial Inflammation

If the injury is very recent - use a good quality cold pack and Arnica Pain Relief Cream. If the injury has been going on for awhile, then skip to Step 2.

Injured soft tissue (muscles, tendons, ligaments) will receive very little blood flow; this occurs because swelling and inflammatory symptoms restrict blood vessels, compromising the flow of blood in the area. Couple that with the fact that your tendons and ligaments receive little blood flow to begin with, and you can start to see why acute injuries can often turn into major long term problems (chronic). In some cases (such as tendonosis), there is little or no inflammation - in such cases the use of cold is relatively pointless as there is little or no swelling to be dealt with.

If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a quality cold pack to the injury area along with a high quality pain relief cream such as our Arnica Infusion Cream.

If you are experiencing pain and swelling, not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • after any activity that stresses the hip, lower back or upper leg in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
  • If you have a job that requires you to place some demands on your injured hip. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will find a Cold Compress or Ice Pack helpful as well.

Hip Injuries Not Only Affect the Hip - But Can Lead to Other Conditions and Injuries.

Every time we use our injured hip, damaged and swollen tendons and muscle tissue move; when they are inflamed, every movement hurts. With injured tissue, pain happens - sometimes a LOT of pain, so we try not to move it. So, when doing something we need to use our hip for (getting dressed, driving, stooping), we start to use the knee and lower back more. Eventually, we use the other hip more and more - or even use our knee instead of our hip for a lengthy period of time. Muscle imbalances result, placing tension on bone and softer tissues - leading to a misalignment within the mid and lower body. The extra stress on the opposite side of the body will also result in weakness and fatigue of the tissues - increasing the chances they can eventually be injured or gradually degenerate. This is a textbook example of how you get an overcompensation injury. In some cases, physical therapists warn that overcompensation can be very dangerous as it will affect gait or alignment on a permanent basis.

An injury to one hip muscle or tendon can easily lead to straining in other areas - just due to the pain response in your biomechanics - making an overall recovery hard to initiate. For example, a serious muscle strain in the right hip can often lead to over-straining of the left hip. Most commonly, the injury occurs on the dominant side, so the risk of straining the other (weaker) side increases. The longer the injury (and corresponding pain) persists, the greater the chance that you will sustain more strain from overcompensation, eventually leading to issues in those areas as well. To minimize potential secondary injuries, know that it is important to deal with your injury quickly and completely.

Truly, hip injuries can become a vicious cycle - which is why it is absolutely critical to focus on methods that improve and strengthen the injured tissues in a timely manner. The faster you truly heal, the less chance you have of sinking into a downward spiral of re-injury, scar tissue growth and overcompensation injuries.


A Note On Hip Pain OverCompensation

Since you are reading this, you probably know that serious hip injuries do not just disappear. Everything in the human body is connected. An IT-Band injury can lead to other injuries over time if not treated properly. For example, the knee of your opposite leg may start hurting due to increased strain on it while you are trying to walk. On-going pain in this area could potentially result in long term (chronic) knee pain.
